Patient Care Management Systems in Wayne County works to become the safety net for Wayne County citizens at every stage of life. We help individuals and families achieve better health at less cost. Our range of services is larger and more comprehensive than many people are aware.
The office of Patient Care Management Systems provides administrative support, enrollment, eligibility and outreach services to the HealthChoice benefit plan which subsidizes health care for small businesses; the Senior Prescription Savings Program that gives up to 50 percent savings on necessary medication and MedBasic, (formerly Adult Benefit Waiver Program) a coverage option for the neediest citizens. We help children enroll in MI Child, the state's health insurance. This summer we roll out plans to help low income citizens with additional health care and dental needs.

Check out our two newest programs within HealthChoice. These include DentalAssist, limited dental services for the indigent with a $4 co-pay and CareFirst Plus, a health plan for indigents that includes annual physical exams, immunizations and urgent care, with services carrying a $5 to $10 copay.
