Wayne County Health Choice - Resources

BasiCare

BasiCare is a HealthChoice program that provides limited medical benefits on an outpatient basis for those individuals who are in transition and have no other health coverage.

Basic Coverage - Covered services include annual physical exams, diagnostic and treatment services and immunizations for a $3 co-pay.
Lab and X-Ray - Covered only if ordered by a physician for diagnostic and treatment purposes. Prior authorization is required.
Pharmaceutical - Coverage is limited. It includes $5 generic and $10 brand-name drug co-pays.
Urgent Care - May require authorization by a plan administrator for coverage. Co-pay $5 per visit.
