Wayne County HealthChoice provides comprehensive health care benefits through an extensive network of metro Detroit's most respected physicians and hospitals.
These physicians and hospitals are affiliated with three managed care plans: Community Care Associates, Inc. (C-Care), Midwest Health AKM and Pro Care Plus, Inc. You select the managed care plan that best meets your needs based on which doctors and hospitals participate with the plan, as well as benefits and services offered. Once enrolled in the program, your health care needs are managed through the participating physician(s) you select.
Community Care Associates, Inc. is a medical service provider with the HealthChoice of Michigan Program. We are dedicated to providing high quality care in an environment that respects privacy and assures patient dignity and member satisfaction. High quality care and service for our members means coordinated health care and services within an integrated delivery system. The ultimate, compelling goal of Community Care Associates, (CCA) Inc. is to proactively and effectively accommodate the needs of our customers.

Midwest Health AKM is a medical service provider with the HealthChoice of Michigan Program. Midwest Health AKM is an affiliate of Midwest Health Plan, a state licensed Health Plan serving over 70,000 residents of Southeastern Michigan that attained NCQA (National Committee for Quality Assurance) “excellent” accreditation. This accreditation is the good housekeeping seal of approval for managed care plans. This accreditation demonstrates the emphasis Midwest Health AKM and its affiliates place on quality care. In accord with the HealthChoice of Michigan Program benefits, Midwest Health AKM provides a continuum of quality health care services to enrolled subscribers and their families (commonly referred to as members) residing in Wayne and surrounding counties.

Wayne County HealthChoice and Blue Care Network of Michigan are teaming up to offer a health care program for the next generation and beyond. The Wayne County HealthChoice Young Adult and Tempro programs provide individuals, ages 18 to 55, with affordable medical benefits through Blue Care Network, a health maintenance organization affiliated with Blue Cross Blue Shield of Michigan®.
