Patient Care Management System in Wayne County works to become the safety net for Wayne County citizens at every stage of life. We help individuals and families achieve better health at less cost. Our range of services is larger and more comprehensive than many people are aware.
The office of Patient Care Management System provides administrative support, enrollment, eligibility and outreach services to the HealthChoice benefit plan which subsidizes health care for small businesses and the Wayne County Patient Care Management System Adult Benefit Waiver Program (ABW) which is a coverage option for the neediest citizens. We help children enroll in MI Child, the state's health insurance. This summer we roll out plans to help low income citizens with additional health care and dental needs.
