Who qualifies?
Employers with headquarters in Wayne County can increase productivity, reduce absenteeism and decrease turnover by extending HealthChoice, a managed care program with comprehensive benefits, to its staff.
Small business owners, workers and HealthChoice share the cost of quality care. HealthChoice is one of the oldest three-share benefit plans in the nation, and it is continuously upgrading to meet its members' needs.
How it works:
Employees pay as little as $58 a month for premiums, which cover board certified or board eligible physicians in pediatrics, internal medicine, family practice and ob/gyn. It also includes urgent care, lab work, x-rays, prescription drugs and limited hospital stays. Employers can purchase supplemental coverage for vision, dental, physical therapy, substance abuse treatment, durable medical equipment and additional inpatient hospital days.
How employers qualify:
- Submit names of at least two employees who are eligible for coverage.
- Show that employees were not offered health care benefits for at least three months.
- 50 percent of the work force must earn less than $14.50 an hour.
- Maintain a Wayne County headquarters.
