Wayne County Young Adult Program (YAP)
Wayne County HealthChoice and
Blue Care Network of Michigan are teaming up to offer
a health care program for the next generation and beyond. The Wayne County HealthChoice
Young Adult program provides young adults, ages 18 to 30, with affordable medical
benefits through Blue Care Network, a health maintenance organization affiliated
with Blue Cross Blue Shield of Michigan®.
The Young Adult Program is a two-share managed care plan. This means you share the
costs with Wayne County HealthChoice for Blue Care Network’s benefits. As a program
subscriber, you will select a primary physician to manage most of your health care

Who's Eligible?
To be eligible for the Wayne County HealthChoice Young Adult Program, you must
- Be 18 to 30 years old
- Live in Wayne County
- Be without any other health care coverage
- Be employed (full or part time) and/or be enrolled in a college, university or some other institution (full or part time)
- Provide a letter of enrollment for verification
- Provide two consecutive pay stubs. Annual income must be below $30,000 (if applicable)
- Provide proof of student enrollment (if applicable)
