The Michigan Department of Health and Human Services (MDHHS) determines the quantities and the delivery schedules for all supplies. Healthcare Practice Vaccination Request

A "Phase 1A Healthcare Practice" is defined as a professionally managed setting that is engaged in the delivery of healthcare services by paid and/or unpaid persons who have the potential for direct or indirect exposure to patients or infectious materials. This form is not intended for Long-Term Care facilities.

Out-County providers who meet this criteria are eligible to request vaccination through Wayne County Public Health and are asked to complete the request form in the link below.

Priority Groups

The Centers for Disease Control and Prevention and the state of Michigan have established a prioritized list of who will get the vaccine and in what order.

The priority groups may be revised by the state of Michigan. Wayne County will follow MDHHS guidance on this distribution strategy and priority group classification.

Immunization Sites

The Wayne County Public Health Division is responsible for the administration of the vaccine to the public. With this charge, public health teams, including our public health nurses, logistics teams, and necessary security detail will be on site to ensure a safe and smooth process. The current locations will be listed and updated here.

Wayne County is currently in Phase 1A of the distribution schedule and no public sites are released at this time. Please visit again once it is announced vaccine is available to the general public.
