Service Connection

Description
Social worker assists with service coordination and connection to community supports for mental health and substance use conditions, medical needs, housing, employment, education, clothing, food, transportation, public benefits, or other needs as identified during the biopsychosocial assessment.

How it's used
To connect client with needed resources or arrange acceptance into an alternative sentencing court such as veterans court, drug court, mental health court, or job court.
Note
Due to coordinated entry policies within the emergency shelter system, social workers cannot currently coordinate emergency shelter placement on a client's behalf. If client is in need of emergency shelter placement, they must contact the appropriate Coordinated Entry Agency based on their location.

Detroit/Highland Park/Hamtramck
  • If client has access to a phone, call CAM at 313-305-0311.
  • Clients can also walk-in to any emergency shelter in Detroit. Operation Get Down, located at 10100 Harper Ave, Detroit, MI 48213 may be best equipped to provide emergency shelter for justice-involved individuals. Other shelter locations are listed on the CAM website.

Out-Wayne
  • If client has access to a phone, call Wayne Metro at 734-284-6999
  • Clients can also walk-in to one of the emergency shelters listed on the Wayne Metro website.
