Documents And Information Necessary To File A Motion

  • Judge or Referee Copy of Motion
  • Case Number
  • Plaintiff Name Vs. Defendant Name
  • Judge Assigned to the Case
  • Notice of Hearing
  • Proof of Service
  • Motion Praecipe
  • Applicable Motion Filing Fee
  • Completion of Forms

Forms must be completed prior to filing. When filing in person, your paperwork must be complete and in order for the deputy clerks to process the filing. Forms must be completed for filing prior to the close of business, 4:30 p.m. Forms submitted by mail that are not completed and/or do not accompany the required statutiry fees, will be returned.
